Who to Contact About Your Care
Your Stride, LLC is not a medical provider and not a pharmacy. Our support team provides non-clinical help only and cannot give medical advice, dosage instructions, or pharmacy counseling.
Questions about your treatment
Questions about your diagnosis, treatment plan, dosage, side effects, drug interactions, or whether treatment remains appropriate must be directed to the licensed healthcare provider responsible for your care. If you are an active patient and need to reach your provider, call us at (469) 559-1589 and we will connect you with your provider’s care team.
Questions about your medication
Questions about how a medication was prepared, how to store or handle it, the condition of a shipment, or pharmacy counseling should be directed to the licensed pharmacy that dispensed your prescription. The pharmacy’s contact information appears on your medication label and shipment paperwork.
Privacy and Terms Requests
To submit a privacy rights request, exercise a right described in our HIPAA Notice of Privacy Practices, ask a question about our Privacy Policy or Terms and Conditions, or raise a billing or cancellation question, contact us by phone or mail using the details above.
